I have been leading a large number of BA's and PM's in enterprise-wide integration efforts at a very big company. Part of my job is analyzing dozens of functional specs and filtering them down to their essentials. This experience was been terrifying. My worst of fears has been confirmed: Rigorous analysis is rare - very rare. And as a result, for non-trivial systems, most design is primitive and disjointed. And as a result most software is primitive. Let us pray for all who write end-user documentation.

Sure. And my experience has been mixed: both fantastic and horrible,
with little middle ground, but I'm also a manager by tendency and as a
result am highly critical of systems that I sense are nonfunctional.
