Her survey appears intended only for those who TEACH tech writing, something I have only done informally. Much of it is addressed to providing details about my institution and the courses where I teach. I was unable to complete the survey in any helpful way.

You might want to revise the survey to account for the fact that not all
professional tech writers have degrees.
