Well, in the case I mentioned I know the back story. My brother's a part of that newspaper. The headline writer (NOT my brother) was indeed trying to be 'sensitive" and missed the mark. "Hearing-impaired" as a euphemism for blind is clearly an error. Journalism is made of errors, but (according to an old newspaper maxim) "Yesterday's newspaper is wrapping fish."
