In Word (or in OO), can you set up the PDF to display the bookmarks on opening the file?

Yes, confirmed, Word 2010 also does all that.

Pardon my naiveté here, but shouldn't Word do this?

I can do all three of these things with Open Office without using Acrobat at all.

In fact, I just opened a random Word file in Open Office, created an internal link, created a TOC, and exported a PDF that has bookmarks and hyperlinks (both internal and from the TOC).

That said, if you do need Acrobat, I think they still offer a free trial, so you could check it out before committing to a purchase.
