Here's guidance from the U.S. Postal Service and
the Associated Press:

The Associated Press Stylebook reads:
Use all-caps ZIP for Zone Improvement Program,
but always lowercase the word code.
Run the five digits together without a comma,
and do not put a comma between the state name and
the ZIP code: New York, N.Y. 10020.

U.S. POSTAL SERVICE
In its booklet 'A guide to business mail preparation'
the USPS follows 'ZIP Code' and 'ZIP+4' with the registered
trademark symbol (R-in-a-circle) on first reference.
