I have been doing books for commercial publication, but interestingly
enough, I have always used my own name, address, and telephone number
when creating illustrations, such as sample forms of various kinds.
Readers often call, somewhat incredulous when I answer the telephone
because they can't believe that the addresses and 'phone numbers in the
book are actual ones.
