You write:
>However, before I start, I would like to make it
>clear that I hate indexing more than I hate many, many things and,
>therefore - while I want a complete and useful index as an outcome - I
>don't want to spend much time doing it (I'd rather spend the time >pulling my own fingernails out - it's more pleasant).
In this case, why don't you hire an indexing contractor. They will do a
better job, and you can go off an pull out your nails in peace. <grin>
They are not that expensive, and their output will be 1000 times better
than what you end up with. Your readers will be much better served.
