Well, I haven't done a PhD (yet), but I did put my M.Ed. thesis online.

The former in my case. The biggest problems I had was that how to ensure
that everyone could read my paper - in the academic community, many are
still using browswers such as Lynx although I don't have a handy statistic
to back this assertion up.

I had also used the International Phonetic Alphabet which caused a real
problem. In the end, I had to use a different prononciation system.

It took me quite a while deciding on the logical order of my paper. I.e. the
credits page, the dedication, and all that. To simplify things for the
reader, I made every link available on my table of contents page.

I debated whether I would put footnotes at the bottom of the page or as a
separate (small) page. In the end I opted for latter, although I'm not sure
if this was actually the better solution.

Anyway, my dissertation was for a Master's in Teaching English as a Foreign
Language and is entitled "A comparison of reading strategies in a
computerised reading situation and in a non-computerised reading situation"
(yeah, one of those long-winded titles).
