Use the STYLEREF field code in the header, with a reference to the heading level you want to appear. For example:
{ StylerRef "Heading 2" }
will show the last text formatted with heading level 2. You can use a switch to specify the next text with that format. You can use any paragraph or character style. Word's help for STYLEREF field gives more information.
