The credit to solving my problem goes to David Egyes of NDS Technologies,
Ltd.

[Egyes, David] The method I'd recommend is as follows:
1. In Word, do File > Print and choose the Acrobat Distiller driver.
2. Select the 'Print to file' checkbox.
3. Open the Properties window and go to the PostScript view.
4. Choose Optimize for Portability (as opposed to the default
Optimize for Speed).
5. Click OK and specify a name for the .ps (or .prn) file. Distiller
recognizes the .ps suffix, but you can create the file with either suffix
and then change to .ps in Win Explorer.

Once you've got your PS file, you open it in Acrobat Distiller and
it is processed into a PDF.


In addition to the above instructions, I eliminated bitmap compression
options in the Job Options>Compression screen of Adobe Distiller. It is also
important to note that I previously prepared the .PDF file directly from the
Distiller Assistant.
