Have you tried that mouse that works independent of the surface? It has a
gyroscope so you can use it in the air. I read about it last year but never
talked to anyone who used it.

You could also combine any input device with an audio command process which
will click for you from verbal commands. That way you only have to the
shoulder movement without the wrist involvement that clicking causes.
