I got a manual recently that had the reference card in heavy
stock right inside the cover. It was visible to me immediately, and I liked
it a LOT. Also, from a user's standpoint, I can have the manual laying on my
desk, and just open the cover and refer to the card. Easier than having to
pick up the manual and open from the back. (I suppose you could remember to
lay the book down on its face, but that's silly.) I choose to keep the card
in the manual, so it doesn't get lost. I vote for your initial feeling.

Subject: Cards of shortcut keys
Hi all,
Our company receives a good amount of customer
feedback through our Customer
Service and Sales departments. For the next update
of the user guide, we are
including a card of shortcut keys that the user can
tear out (perforated)
and stick by their computer. If anyone has created
one of these before,
where did you place it--by the front cover or by the
back cover? Why did you
pick one location over the other? My initial feeling
is that we should
include it by the front cover, where the user would
likely find it sooner.
