Wouldn't this depend on what you are writing the manual for and what
kind of software you're documenting? Instead of trying to fit the
manual to a mold, consider making a new mold for the manual. In other
words, think first about who you want to write it for (not their
titles, such as "programmer," but their functions), and then what they
will do with your document (train others? configure software for
inhouse? for clients? troubleshoot? test the application?).

Then, once you understand why you are writing a manual, and once you
are certain you understand the technology (very important for a
technical manual), find some similar products and look at their
documentation.
