Don't know about your item (2), but have you checked your Windows fonts
manager (assuming you are using Windows) from the control panel? It is
possible that you have a truetype version of the font installed as well
as/instead of the postscript version. Sometimes a font can have the
exact same name as another (or a different rendering of that font). I
have noticed this with variations of Garamond and Bernhardt for example.
Don't know if this helps at all, but thought I'd mention it.
