Some time ago I asked how people used .PDF files. While I
haven't had a chance to do a full analysis of the results, I got
about 75 responses, all but about 3 of whom hated .PDFs for
various reasons. The 3 that said they liked .PDFs didn't say
anything about *using* them, but only how convenient .PDFs were
as a way to ship documents.

ok, so my company has basically acknowledged that we use .PDFs
because they're cheaper to distribute than printed documents,
and faster and easier to produce. However, we're still not to
consider them online documents, but "printed." so that a lot of
the print conventions still apply, even though those conventions
may be totally wrong for the way the documents are mostly used.

We have gone to solid sequential numbering - that is, the first
page after the table of contents doesn't start at 1 any more,
but continues sequentially from the last ToC page. The main
reason for this is that you can't get the Acrobat Reader page
numbers to start at 1 again, and people were getting confused by
Reader saying you were on page 47 when the page number on the
"printed" page said it was page 44.

you know, my life would be much easier if we'd just acknowledge
that these are online documents,and I could format them as such!

My questions are:

If you use .PDFs, do you include a title page and table of
contents in your document?

If you do, do you start your first text page as page 1, or
number sequentially from the ToC?

is it possible to print the "bookmarks" pane, so it could be
used like a table of contents?
