I'll only comment on your first and second questions. Yes, we use TOCs in our documents. The main manual
we maintain is published online, with updates once a month. This manual is in the form of one large book
consisting of 12 smaller books. Each of the smaller books has its own TOC. The index at the end of the
book covers the full manual. We use a "point" page numbering system consisting of the book number,
chapter number, and page number, for example, 8.12.3.

When we print this book to postscript, we print it as one job, but with separate .ps files as output. We
then distill the files to PDF. The page numbering within each file is maintained. The TOC for the
"sub-book" that I just looked at consists of 8 pages, and the first narrative file (chapter) within that
book consists of 70 pages. When I open that chapter, it starts with page 1, the status bar says 1 of 70,
and there are indeed 70 pages in the chapter.

I don't know if this answers more questions than it raises or vice versa.
