My questions are:

If you use .PDFs, do you include a title page and table of
contents in your document?


Yes, and if we want them to be jumped to we place navigation features in the Bookmarks tab.


If you do, do you start your first text page as page 1, or
number sequentially from the ToC?


Depends upon the document. We have docs that run sequential by chapter. Not a problem. Acrobat 5 allows you to add this functionality to your PDF. Chapters can show prefix and restart at each chapter.


is it possible to print the "bookmarks" pane, so it could be
used like a table of contents?

I don't think that you can print the bookmarks.

I work in FrameMaker, so all my hypertext links, Chapter Titles, through Head1 to Head3, tables and figures are converted into links in the PDF file. The Bookmarks make themselves, and the Index and TOC are all automatically hypertext linked as well.
