Not yet mentioned is the organizational cultural attitude of knowledge: both creation and management.

If the focus is on knowledge creation, the management is usually considered a justifiable overhead expense. If knowledge creation is not well understood, likely the management is a bit slippery. If knowledge creation is not valued or understood, knowledge management will be a mess.

Doctors are more and more taking telemed out of the hands of the techies because medicine is all about growing knowledge and the techies would rather focus on widgets. The conflict is not resolvable until techies get it through their heads that its the knowledge creation that keeps things moving forward. Software and hardware only are facilitation tools.
