This sounds similar to a problem I had awhile back (it's been well over a
year, I've slept once or twice since then, so I may not remember all the
details). I had to define the specific paper size in Acrobat.
We had a weird paper size defined for the Word doc (our printer would
duplex print, but not the special sized paper we needed, so we had the paper
defined as landscape with the appropriate margins, then had the printed
manual trimmed to size. Convoluted, but it worked, since we produced very
few hardcopy manuals and mostly distributed it via pdf), and it caused all
sorts of grief when we did the conversion to pdf. Creating a custom paper
size in Acrobat solved the problem, and it printed to the correct paper
size. At least onscreen it was the correct size.
Another thing you might check is the Print tab in Word's options dialog.
Uncheck the Allow A4/Letter paper resizing option.
