Documentation Change Requests. Ugh!

At my last employer we had to use DCR's, which were in the same format as Engineering Change Requests. They involved documenting exactly what changed in the document (including page numbers for major changes) and then they were submitted for sign-off by the responsible parties. They were generally in the form of 1/2 sheet forms with four carbonless copies, and were produced by hand. (Ugh again) They were duly submitted to document control and from there, I can't say what happened to them, beyond knowing that they were filed away.

The main point of these is that in regulatory environments (the product I was documenting was a RF product that was certified by the FCC) you have to keep careful track of a all the changes you make so that there's a paper-trail if something goes wrong and the product fails to operate as certified.

That's not much specific help, but it's a start.

Does anyone out there deal with Document Change Request, as a formal process. If so, do you use a set template for document change request submissions, do you have any presentation materials or documented processes that you would be willing to share. And please, feel free to share your experiences with dealing with Doc Change Requests, the good and the bad.

I've never done this sort of thing before, but a client is asking for such a thing...

Also, I would be willing to share any information I gather with others.
