If you have your section set up to use all three header-footer pairs, but the document contains only one or two pages, you *cannot* define all the header-footer pairs--your file needs at least three pages to define all six headers and footers. (In a multiple-secton file, each header and footer can be defined to duplicate its corresponding definition from the previous section, but each has to be defined separately.)
I had forgotten this fact!
If you have a two-page chapter, add the third page anyway even though it will be blank except for the h&f. It will ensure that the next chapter begins on a right-hand page. If you don't want the next chapter to begin on a right-hand page, add the third page, create the h&f, then delete the page. The h&f setting for the third page will remain.
