The "c" and the "s" with lines over the top are abbreviated Latin...
"c" for "cum" (with) and "s" for "sine" (without).

Google around for "medical shorthand" or "medical chart abbreviations"
and the like - I did a quick and dirty search and found this page,
which is good but far from complete:
