Bonnie, I strongly recommend that you do a small but representative sample
job first, then negotiate a rate for the remainder of the work. It could
be one small manual, or a skeleton containing a cover, TOC, one chapter,
one appendix, glossary and index.

At the end of this you'll have a working process, a mostly complete set of
templates, and sample output that the client can verify as fulfilling
their requirements. Even better, you'll have a good idea of how to
calculate a per-chapter or per-page rate.

Both sides are better off. You don't have to worry about the job blowing
out due to unforeseen difficulties, and the client knows almost from the
start exactly what they'll get, how long it will take and what it will
cost.
