I'll second the recommendation of the indexing chapter in
"Read Me First". It's really a good introduction. And about
a half-dozen years ago, when our pubs group was *much*
larger, we contracted with Lori Lathrop, a prominent
professional indexer, to run a 2-day indexing workshop,
and she also recommended that chapter.

I don't do professional indexing but our
documentation department puts a high value
on it. Our style guide has a chapter on
indexing, duplicated in our third-party
generic version (READ ME FIRST) that has been
complimented by a professional indexer on
Amazon. You might want to take a look at it
for some tips.
