For what it's worth, I have it listed in an
older one than that: the Fifth Collegiate Edition,
1943, including a section of "new words" (like
"jam session" and "mechanize") up to 1941.
"And/or" wasn't a new word, though. It's right
there on page 40 of the body of the book, between
"andiron" and "andro-".
