Same here.  IMO, the only reason to make a "print formatted" PDF and
not make the links stand out is if you're planning to send it to a print shop
to produce printed docs.  And it shouldn't be all that hard to use condiional
text or plain old search-and-replace for styles to switch the master doc
files bsck and forth even if you don't use a multichannel publishing tool.

If it was up to me, I'd keep the index for PDF viewing too, and make all
the entries jumps.  I'd have to think about whether the whole entry ought
to be colored and underlined.  Maybe just the page numbers...
