Basically, you have to call an office in Wash., DC. You'll get
an answering machine on which you'll leave your name, address,
and a request for the specific material you need. About 3-4
weeks later you'll get the forms through the mail. Fill out the
forms, write a check (usually around $20.00), enclose both with a
copy of the item you're getting the copyright for, and then mail
it off (this is for copywright; I'm not sure about trademark).

If you send a response to me, I'll see if I can dig up the phone
numbers for you. The whole process is relatively painless.
