This originated before IBM. It was a requirement in classified military
technical manuals. You were required to account for every page--even
blank ones. They wanted to be sure the page was blank on purpose and
not because someone messed up. I can remember it in specs back in the
late 50's. IBM was into computers then, but not manuals. They only
made mainframes and there were no software manuals to speak of.
