Why not include the document code as part of the page
numbering pattern in each document the same way you
would include the word Appendix, and have that code
brought into the index at the same time that it pulls
in the page and section number?

If it needs to be hidden in the document but displayed
in the Index, just make give the code a color
attribute of white and it won't be seen.
