There are three possibilities that come immediately to mind:

1. Someone has a specific person in mind and needs to demonstrate
to someone else that there's no alternative candidate with equivalent
skillset. This could include the H1B candidate that has been
suggested, but could also include the hiring manager's brother-in-
law, or someone who has the exact skillset the hiring manager wants but whose salary requirement is several grades higher than
what the company has budgeted and must be justified to upper
management.

2. The company is trying to replace an individual with a unique
skillset who left and has assmembled a shopping list for someone who is an exact clone of the departed employee.

3. The company has had a long chain of people who almost succeeded
in doing this job, and has assembled a shopping list that consists of every skill they have identified as something a previous employee
lacked.

In case 1, there's probably no point in applying because the person has
already been selected. In cases 2 and 3, if you only lack one or two of
the items called for, you might have a chance.
